I am writing to request a favor from you. Over the past few months, I have been facing increased anxiety levels due to a combination of personal and work-related factors. I, therefore, need some time off from work for the sake of my mental health.

I have been feeling mentally exhausted and stressed. As a result, I fail to focus on my work. My productivity has declined, and I have been unable to fulfill my job responsibilities with the same dedication as I did before. I believe taking some time away from the stressful environment of the office will be helpful. I’ll be able to relax for a while and restore my mental health.

I would like to discuss a suitable time with you for a two-week leave. Please let me know when you will be free to discuss the matter.
