Medication Record Sheet

Sometimes you need to give a patient certain medicines for specific conditions under observation, so you can onion if and when the desired result was achieved by the medication e.g in seizures etc. A medication record sheet is the log that makes this feasible. A medication log is written for individual patient while monitoring the medicine given to the respective patient. A medication record sheet holds the following record :

  • The name of the patient
  • The age of the patient
  • The gender of the patient
  • The name of the doctor who prescribed the medication
  • The name of the medication
  • The purpose of the medication
  • The dose of the medicine to be given
  • The time the medicine is to be given
  • The number of times per day the medicine is to be given
  • How the patient responds to the medicine in the first few hours after taking the dose
  • How long it took the medicine to produce the desired result
  • The tenure a medicine is to be taken for if the medicine is to be taken before, after or during meal


medication record sheetIf the condition of the patient is not serious and he/she is not hospitalized,  this record sheet can be maintained at home by the patient himself but this decision depends upon the opinion of the consulting doctor. The medication record sheet is the report that serves as a legal record of the drugs administered to a patient at a facility by a health care professional.
